Description
Vita Valens is a Brooklyn community-based organization (CBO) with roots across the boroughs, striving to improve the physical and emotional health of New Yorkers. We believe that genuine and compassionate connections are key to providing health support. Our approach recognizes that physical and emotional well-being are intrinsically tied to the social determinants of health. We focus on increasing access to health information, social isolation and loneliness, and facilitating connections to health care services and resources. As part of New York State's 1115 Medicaid Waiver, Vita Valens is helping to identify and address health-related social needs (HRSNs) by screening Medicaid members and directing them to community-based resources and care supports.
The Social Care Network Screener/Navigator will join the Vita Valens team in screening members for HRSNs and coordinating referrals to appropriate community-based organizations. The Screener/Navigator will guide participants through the process of accessing support resources.
This role is primarily onsite at provider clinics with some opportunity for remote work. The location of primary sites will vary across Brooklyn, Queens, and Manhattan.
RESPONSIBILITIES
- Conduct social needs screenings and assessments to identify participant needs and determine appropriate resources and referrals
- Assess eligibility for enhanced care services and community-based programs
- Send and manage referrals through the Social Care Network (SCN) platform, ensuring members are navigated to the appropriate community resources
- Provide education on benefits and resources available under the NYS Medicaid program
- Follow up on open cases to confirm successful member navigation and close cases within expected timeframes
- Maintain accurate and timely documentation of all screenings, referrals, and member interactions in accordance with protocols
- Provide professional, culturally sensitive support to members through direct in-person interactions, phone calls, messages, and ongoing communication
- Meet performance, attendance, and productivity expectations, contributing to team goals and overall program success
- Engage in consistent and prompt communication with the team, participate in scheduled meetings, and provide updates on progress and assigned tasks
- Complete all required SCN trainings and certifications, including HIPAA and Mandated Reporter training
- Ensure full compliance with HIPAA and confidentiality standards, safeguarding all protected health information
- Adhere to Vita Valens, NYS Medicaid, and SCN-specific policies and procedures
REQUIREMENTS
- High school diploma or equivalent required
- Must be available to work in-person; opportunities for some hybrid work as well
- Excellent communication, interpersonal, and active listening skills; bilingual skills are a plus
- Technological proficiency with data entry and record keeping
- Attention to detail when entering data and documenting interactions
- Ability to work independently and as part of an interdisciplinary team
- Must maintain professionalism in sensitive and confidential situations
